The presenter was a<br> member of the Command Investigation team, and they discovered a<br> shocking lack of coordination and communication between the disabled aircraft and its pilot and the squadron ready room, in addition to maintenance oversights and questionable decisions across the board. After publicly releasing the findings of the investigation, lawsuits were filed against the government<br> by the family of the victims.
